DATE OF ENTRY OF DEFAULT what date do i put? also date of publication is wrong?

On the top of the default document is a file stamped date. If that file stamped date is on the Default Order then that is the date of entry of default. The last six words you wrote do not constitute a question even though you put a question mark at the end of the sentence. So, I cannot respond to that conglomeration of words.
