I need a blank form for an insurance carrier. I
am an administrator and I just need to file this form for my father's estate and can I please get someone to email me this correct answer to this form?

There is no actual 13100 form. Probate Code Section deals with Small Estate Administration. Most institutions have their own form for this. There is statutory language contained in the probate code which must be included in your affidavit. If you have further questions, please feel free to contact my office.
