My cousin has an open family law case in the state of CA, Ventura County court. She just passed away and I cannot reach the attorney for her case (all numbers are disconnected). Is there a form for me to complete to notify the judge overseeing this case regarding the death of my cousin, the respondent?

Her attorney should be notified and should be handling it. Have you tried using the attorney information at the California State Bar? Here is a link: http://www.calbar.ca.gov/
Depending on the status of the divorce case at the time of her death, the court may have retained jurisdiction over any property issues.
