I am filling out the civil case information sheet and I am wondering if I need to put the name that appears on the original court order for my ex-wife, or should I put her maiden name, since that is what she goes by, even though she has remarried?

Use both names:
"Carmen Jean LaMotta [married name, as shown in the last order] also known as Carmen Schmuck [the name she's using now]."
